Todd Sobol, MD, FACP, CMD has over 30 years of experience managing patients in nursing facilities as well as in the acute care hospital and outpatient private practice setting in Dayton, Ohio. He currently serves as Chief Medical Officer for Personalized Health Partners. Dr. Sobol serves as a Regional Medical Director for the Regional Rapid Response Assistance Program supporting the Ohio Department of Aging. He has worked as a Regional Medical Director managing Value-Based Care products including ISNP. He is a Past President and active member of the Board of Directors of the Ohio Medical Directors Association (OMDA). A regular speaker at The Society for Post-Acute and Long-Term Medicine (PALTMed) national conferences, he also works on the teaching faculty for the Core Curriculum for Medical Direction for CMD Certification. He serves on the American Board of Post-Acute and Long-Term Medicine (ABPLM) Board of Directors. He is on the Advisory Board of CareBand, developing advanced location tracking wearable technology for Dementia management.
